Did you ever check the choke. If you have to hold the gas in, then likely you need to set the choke to be on because it won't idle when the engine is cold.
as for the smoking, it depends on the color of the smoke. Blue smoke is oil, likely caused by blow by due to bad rings. The fix for that is to replace the rings, which means rebuild the engine.
If the smoke is white then it is likely water/coolant. Either this is because there is condensation in your cylinders/exhauts that is evaporating because the engine has sat for a while and is cold, or you need a new head gasket.
