Nice looking truck! For the filler neck question, I wouldnt do less than a 15 degree angle off of horizontal. I tried it on my truck with less, and fuel wouldnt flow fast enough, kept backing up and spilling out, so i ended up cutting a hole in my bed floor and going almost vertical with it.

I just did mine in the passenger side door jamb with the filler neck made from exhaust tubing that runs about 4" back past the jamb at just a slight angle, then used marine grade rubber fuel hose to run it down into the tank.
I also tapped in a breather line from the filler neck down to the tank so the fuel would flow like it should without it burping back out and it works flawlessly.
The nice thing about the fuel hose i chose to use was that it was steel reinforced and molded fairly well.
