CAD or SOLID WORKS HELP?

I have something I need help with from somebody with either a CAD program or just expierence with drafting in general. I have posted below a template that Im plan to make but somehow my numbers dont all add up and Im not sure where my math got off track. But if someone could make this up in a CAD, similar program, or catch my mistake then please help.

I would like to be able to hand this print to a machinist friend of mine with no errors on the print that way Im not wasting his time. thank you to any one willing to lend some help.
