I had this problem a few months ago. It was my right rear wheel brake cylinder was seized up and was leaking a little not much at all, just enought to let some fluid out and some air in. Might want to check ur both rear brake cylinders. Havent had a problem since then. It took a about 2 hours to get all the bubbles out tho.

There are a few easy things that can cause pedal fade like you are talking. First if you have a loose wheel bearing it will push the pad back while driving and when you first hit the pedal it pushes the pad but not enough to stop so when you hit it again its close enough to stop. The other is the master is going out, most have a valve that keeps a little bit of pressure on the pads...like 3 pounds. First thing i would look at is the wheels bearings then the master.
