Vibration in truck

Your truck would not run very good if the belt was off one tooth. If it was your flywheel being out of balance it would have always done it and not just started all of a sudden.

Hmmm, it has been doing it for few months at least and seems to be getting worse? What could it be?

Do engine mountings ever need replacing?

yes, mazda motor mounts dont last forever, i would seriously check them out. if it does it at idle, let it run and start trying to figure it out. I can tell for a fact its not going to be your flywheel. most garages would charge you the hours for the flywheel job but only replace a motor mount everyday of the week.
